The Power of Healthy Mind and its Impact on Chronic Disease and Comorbidities
This training seeks to educate and empower home care workers in their role as a member of an integrated team. Participants will gain in-depth understanding of how depression affects elderly clients and its impact on overall health well-being. The training emphasizes how the lack of adequate care, management, and physical follow-up often leads to poor health outcomes with this vulnerable population.
To educate, train and empower the home health aide in their role as a member of an integrated care team. In this role, the home health aide will serve as a liaison between the patient, the licensed agency clinician and the patient’s primary care physician. The home health aide will assist in the management of depression and improve quality measures associated with falls, compliance with prescribed medications, avoiding unnecessary hospitalizations, emergency room visits and combating feelings of loneliness and isolation.
