Care Management Skills for Interdisciplinary Clinicians (Webinar)

This course supports transitional care and care managers to reduce avoidable hospitalizations and provide principles and best practices for all care managers. This free course is a great chance for any clinician to enhance their population health and care management expertise and skill set.

This will prepare Registered Nurses to take the American Nurses Credentialing Center (ANCC) Case Management Exam and any clinician to take the Commission for Case Manager (CCM) Certification Exam.

This course is for all clinicians working in Care Management.
